I also have arthritis in my knuckles, and can recommend Glucosamine and Chondroitin, a natural supplement. It has no side effects and is very effective. It takes a week or so to get into your system, so don't be discouraged if you don't have immediate relief. It is inexpensive and can be found anywhere, including Costco and Sam's Club, in large bottles. When my arthritis first flared up, the doctor prescribed a medication which did not work as well. Now, if your arthritis is really serious or is rheumatoid arthritis, then you really need to see a doctor. But this worked for my very minor osteoarthritis. Good luck!

Looking at your diet and exercise can make a huge difference.

I can reccomend two creams you can get over the counter at a Natural Foods store-Arnica and Topricin. Both are highly praised for pain control, though some people get a better result than others.
Also, you may want to try acupuncture as it can be very helpful for joint pain. Several people I know have gotten very positive results with treatment. If you are near Columbia/Ellicott City, I know several great practitioners. Good luck.
Try the supplements with Chondroitin and Glucosamine. They don't work for everyone, and they are only for osteoartritis, not rhumatoid arthritis, but they can be effective. They are from natural sources, and they put back the building blocks that your body needs to maintain your joints. If all your cartilage is gone, then they will not help, but anything that reduces inflammation helps.

Like one of your other responders said, checking your diet can really make a difference- You may want to try a wheat free diet for awhile- Costs nothing, and it might help a great deal- I had an episode of "Sudden Onset Rheumatoid Arthritis" about 7 years ago, and coincidentally found out I was allergic to wheat- It took me a year to figure out that no arthritis drugs were helping me (and I was in fact on steroids to keep it under control!), and when I finally got to removing the wheat from my diet- POOF- all the swelling -went-away-. I was astonished. It's worth trying- every body is different, but this is relatively "easy"- Doesn't involve special pill regimens or exercises!! :) I now no longer need anything stronger than an occasional tylenol (like 1x/month!) to keep the "achies" away.
There are a few studies out there that find that the proteins of wheat tend to collect in arthritic joints- they don't know if the proteins help to cause the arthritis or if they are drawn there after the fact, but many people find relief after they stop eating wheat, because those proteins don't collect there any more.
