Well... I feel your pain - :)
I am too a healthy B cup and after my first I was a EE at first, then down to a D until I got pregnant again and quit breastfeeding at 5 months pregnant with #2... I am now nursing my second who is 8 months and was just sized the other day again as a 32EE, but of course they don't make that size so she put me in a 36D. I did have engorgement when my milk came in, just as bad as the first time, I wore a sleeping bra for a few days when I got home from the hospital. I ended up wearing my same nursing bra's as I wore the first time, the ones that weren't totally worn out.. then went and bought new bras after the baby was about a month old.
I wore non-underwire bras most of the time while pregnant and they were the "full figure" style. I suspect if you tried a different D size they might be better fitted for you as your pregnancy progresses. The lady at Macy's told me that the full figure provide better coverage and they make a lot of styles that have good support and no underwires.
