Hi Mamas! My husband and I will be going out of town. Our kids will be staying with their grandparents while we are away. Does anyone know where I can download a free child medical consent form online? Thanks!

Actually they aren't worth the paper they are written on anymore.

You need to go to your child's physicain and the local hospital, anywhere they might go for medical attention and fill out medical consents there. If a paper isn't notarized and legally filed anyone could say it was written by you and take your child to doctors and you might not even know. Since the privacy laws changed they just don't hold any authority anymore. There is federal law that changed regarding a limited authority type of "permission to treat" paper now too. You both go before a judge and they sign a paper that is legally filed in the court house that gives certain people to seek medical, and whatever other authority you want to allow them.

I have guardianship of my grandkids and attend a Grandparents Raising Grandchildren group. We have an attorney who is on several government committees that writes the new laws and there were over 400 pages of new law last year regarding grandparents. This paper I am referring to is one of the new laws. It allows the grandparents limited authority to do the assigned things in your absense. It shouldn't be expensive but until you revoke it they have the authority you allow.

P. S. A friend of mine who is a professor of nursing had to fly out of town to attend her mom's funeral and her daughter cut her foot on a ceramic tile in my yard. We had a signed letter at the Pediatricians office but not the hospital, we had a permission to treat letter too. And everyone at the hospital knew who we were and the mom because she had student nurses in their hospital every sememster. They knew us too. They wouldn't treat her child and called a judge to order the treatment. They just will not be setting themselves up for lawsuits with all the new laws.

I didn't know they had them. I usually write a letter and include my contact information and all my childrens medical information and a copy of the insurance card.

Just type something up and have it notarized- it doesn't have to be anything formal- just having your signatures notarized is the most important part. Just needs to say you give permission for the grandparents to consent to medical procedures in the event that you or your husband cannot be reached. Also include the dates that you will be gone so it is not a "forever" form.

You can also type "free child medical consent form" into Google and I am sure you can get at least an example of one.

I write the letter and copy the insurance card, and I get the letter notarized. I don't know if I have to, but it's what I've always done, and it's what I've had parents do if I take their children with me on vacations. I've had to use a notarized letter before from another parent when I had their daughter with me on vacation and she got very sick, and the hospital accepted the letter. It was years ago though.

My daughter has just handwritten a note saying that she gives me permission to obtain medical treatment for her children. During a regular appointment at the various doctors offices she has signed a form that they have granting me not only permission to obtain medical treatment but for them to share information with me. I have been with her when she's had to take her daughter to the ER (asthma) at which time she's signed for their records also.

It is best to include a copy of the insurance card but I've been able to manage without one at times. I do have to sign a statement accepting responsibility for the payment if insurance does not pay. Insurance has always paid.

This way I can take her children for medical appointments so that she doesn't have to take time off from work.

Not a direct answer to your question but additional info for other similar situations.

Yep Gamma is correct.
You may find one hospital that will take it and another that says nope.

Remember the Hospital has to stable a child in an emergency.

The best thing you can do is make a complete medical history sheet.
Name of child, date of birth, any known allergies, a complete list of medications including over the counter and dosage, past medical issues, name of current physician, etc.
Also a complete list of phone numbers that they may get a hold of you at.
