Do We Need a New Pediatrician

Hi moms -

My daughter's pediatrician missed diagnosing a UTI that turned into a kidney infection, and I'm wondering if I should switch her to a new doctor. I really like the idea of continuity of care, but my faith has been shaken.

On Tuesday my daughter started throwing up. After the second time I called for an appt, and after checking her, the pediatrician said that she probably just had a stomach virus and not to worry. She threw up 12 times that day, and then seemed to get better. Wednesday & Thursday she seemed fine, although not really herself. Friday she woke up with a high temp (104) so I took her back in and was told that it was just the stomach virus and not to worry - and that she'd probably have high temps for the next 3-4 days. On Sunday she seemed better during the morning (no fever) but was still lethargic, and Sunday night she had a fever of 105 so we called the doctor on call for reassurance (not our regular pediatrician) who after hearing about the high temp and how long she'd been sick, said to take her to Children's, where she was diagnosed with a kidney infection.

The hospital said that the reason that it became a kidney infection was because of the length of time that she was untreated, but that a urine test doesn't usually show infection until it's gotten pretty bad in babies.

I think that if our regular pediatrician had been on call, she'd probably have told us that it was the stomach virus, and like she'd said, our daughter would have high temps for several days and to just wait it out.

Has anyone experienced this? Did it take a long time to diagnose a UTI? I feel like my daughter's doctor really messed up, especially since the first thing that the on-call doctor asked was if our daughter's urine had been checked for infection. Am I being unrealistic and blaming her regular pediatrician for something that she couldn't have detected?

I'm just sitting here thinking about how things could have been so much worse (and 8 hours sitting in Children's ER with a miserable baby is no picnic) and feeling guilty that I just sat back waiting for her to get better while the whole time she was getting worse.

I think a lot of peds would have did the same thing. My friends daughter went through the same situation. She ended up in the ER from a seizure, that is when they diagnosed the UTI. The problem with UTI's it children is that,they can develop into a kidney infection very quickly. I always thought that a temp of 104 or higher in children should be sent to the ER, but I don't know where I heard that.

I tried 3 pediatricians before I found one I respected and felt comfortable bringing my children to. Don't feel guilty for leaving or trying someone new, you are protecting your children. I would also make sure your pediatrician knows that she totally missed that call and risked your daughters health, so that she might be more careful in diagnosing bladder infections other babies in the future. Just leave a voice message or send a note.

I would find a new one. You need a doctor who considers ALL possibilities when you bring your child in with an illness...Someone who does tests "just to be sure" it isn't something else. And who can put aside past experiences with other children and not assume that just because your child has some of the same symptoms as them, that they have the same issue.
Now...finding a Doctor like that is rare! I bring my son to the same pediatrician for all of his well-child visits, but when he has any other problems I bring him to a different Dr almost every time. That way if he did have something like a UTI and one Dr says it is a stomach virus, and I take him home and am not convinced, I can see another Dr for another opinion without having to argue to the original doctor that he must be wrong.
One thing that did happen to my son was during birth, his collarbone was broken. No one knew about it until he had a chest x-ray 7 weeks later and found out that it was already healed. Well, I believe now the broken collarbone also caused his torticollis...my son would always keep his head turned to one side no matter what I did [I tried wedging him in the other direction when he was sleeping, trying to get him to turn it all day long, etc], and I am convinced that this was because it was painful for him to use the other broken side. Since he always wanted his head turned to the one side, he developed plagiocephaly [flat head on one side]. I told my son's Doctor about my concerns when my son was FOUR months. He said "why don't you come back in a month or so and we will see if it has gotten any better". I didn't like that answer, but I didn't see another Doctor at that time for a second opinion because I trusted him. Well, I took him back around 5 months, and at that point he said he would refer me to Gillette Children's Specialty Healthcare to talk to a specialist and see if he needed a Craniocap to correct it. So, I scheduled the soonest appointment available [2 weeks out] and yep, he needed physical therapy and a Craniocap. BUT...it took another month to get in for the imaging of his head, and another couple of weeks before the Craniocap arrived. My son was over SEVEN months old before he got the Cap. I found out that the most effective time to correct his head was between 4 and 6 months!!!! We used the Craniocap for a few months and went to physical therapy...but since we started so late, his head is still not quite normal and he is 16 months. There's no going back. Anyway, I feel so dumb for trusting that Doctor. I didn't feel right but I didn't seek another Doctors opinion. Never again!
So you can see why I think you should switch, and any time you think the Doctor might be wrong... or especially in your case when you KNOW the Dr was wrong, see another one! Your child's health is in YOUR hands

If you feel unsafe with your child in that person care then you should switch but that is a commonly missed diagnosis. and everyone makes mistakes. i am not defending your dr i am just saying that is you "feel" your daughter is not well cared for then switch her, but if you are just concerned that the dr made the wrong diagnosis then there is no need to switch. my 5 year old niece spent 5 days in the hospital with a kidney infection after 3 different dr's failed to find the infection. and the only reason the hospital admitted her was for the 105 fever it wasn't until after 2 days of my sister requesting the urine test that they found the infection. My sister had to request the specialists opinion or they NEVER would have caught it. good luck in making your decision.

I would have just assumed it was a stomach virus, too, but then I am not a pediatrician. With a fever that high, she should have been checked thoroughly when you took her in. A GOOD dr. does not make a split-second diagnosis.
I had a pediatrician diagnose my 4-month-old baby's asthma as a milk allergy in spite of his wheezing and without doing any kind of test. I took him to a different dr. and he ended up needing a nebulizer and 2 medications to get it under control.
If your doctor cannot take the time to consider ALL the symptoms and provide adequate health care for your daughter, you need to switch.

Well, If you are asking the question of whether you need a new doctor then that is your answer. You need to find a doctor that you trust and can ask questions regarding you childs health and not feel like you are getting the run around.

I agree with finding a new ped. We ended up switching from ours once because my 3 year old son was running a temp and coughing really bad, they said it was the croup and gave breathing treatments in office and sent us home. 6 hours later his temp spiked to over 105 and I was on the phone with the ER and they wanted to send an ambulance to our house. Turns out he had pneumonia and the ped missed it.

Hi K.
I do not think you are overreacting I do think you need to find another doctor. You also need to know it is ok to follow your motherly insticts if you think there is more going on then the doctor will look into then get a second or third opion. Good Luck I hope your daughter is feeling better soon
