I would write her a heartfelt note ( I'm a teacher and these mean a lot to us!) and a little something personal. You could get her a nice photo album that she can fill with her photos, or a gift set, or gift card.
There is a daycare teacher that has taken care of / worked with my kids since they have been in daycare (2 months old) that's last day is coming up - she is leaving for college. She has been great with both of my kids (boy and girl twins), especially my son. Actually, the other daycare teachers have commented about how attached she is to him. Anyway, does anyone have any recommendations on an appropriate gift for her? We thought about a picture or pictures - but she has taken a lot of her own.

Depending on how much you want to spend, a digital photo frame or digital photo keychain would be great. You can put some pictures of your kids and then she can add her friends and family. Thats what we did for our sitter when she went to college and she loved it!
A photo and a gift card to a store like Target. Believe me, daycare personnel don't make much money. While I'm sure she thinks your kids are great, money is always needed for college set-up expenses. Targets are in most ciites and have a wide variety of product (even food) she could purchase.
