Red cheeks could be lots of viral things. "Screaming in pain" could mean he broke his arm or foot or dislocated something. So, just spitballing here, perhaps a medical professional of some kind should be involved???
Screaming in pain for the past 4 hours. Very out of character for him. Doesn't attend preschool but sees people at grocery store etc. any ideas?

If he's screaming in pain for 4 hours straight - why have you NOT taken him to an urgent care or called his pediatrician? Why are you coming on-line to ask strangers who don't know your grandson nor can we see what he looks like??
Red cheeks (if they are bumpy or scaly) can be the start of Hand Foot and Mouth.
Red cheeks IF just read like he has been slapped? COULD be Fifth Disease that is making a come back in our country. Fifth Disease is a virus that starts with bright red cheeks. It is a contagious disease - however - once the rash appears - the virus is basically gone.
Your grandson needs to be seen by a doctor. No child should be screaming in pain for hours without being seen.
A great idea would be for his parents to take him to see his pediatrician.

Could be fifth disease. Highly contageous, dangerous for pregnant women. I had it as an adult when my kids did not - most likely place I got it? The grocery store.

My son would get extremely red cheeks at that age when he was teething. He would look pale with very red apple cheeks. Teething was very hard on him...you could tell he was in pain. You should definitely take him to the doctor just to be sure though.

Veruca Salt is right about what she's saying. Also, an ear infection is something to think about. Have you taken his temperature?
Get him to the doctor. And keep him away from pregnant women.
Screaming can have a lot to do with ear infections, fevers can cause redness (and so can many other things). I hadn't thought of fifth disease but that's another possibility. Is he teething? What about abdominal issues - is he constipated or gassy? That makes kids scream all the time.
Of course he doesn't go to preschool - but I think you are saying that he's not in day care so he didn't pick up something that way, right? But yes, he goes out to stores, and the adults in the family bring things home anyway.
You don't say if he has a fever. Have you checked? What have you already given him for pain or fever? Have you given him a teething ring? Is he over-dressed and super hot? What does the doctor say? Every pediatrician has a nurse on staff and a doctor on call in the evenings - use that number and call the medical people who know the child the best. They also know what's "going around" that other kids have. They are going to be much more help than a bunch of random strangers on the internet. This is a good forum for general advice but not for acute situations like the one you're in. If you're a grandparent who is out of practice in dealing with infants, it's time to get back into the game with a call from his parents to the doctor, or by you if you are in charge of him for the time being.
Red cheeks could be the sign of many things. Maybe his skin is just chapped. Maybe he's having some type of allergic reaction. If he's in pain, he needs medical attention

Sounds like he needs a trip to the ER. Why wait to see if he stops screaming in pain?? He's in pain, take him, right now, to a doctor.
