Rectal Bleeding and Bloody Poop in Diapers

Last night was traumatizing for us. We had to bring our 5 month old to the emergency room at 3am. I heard her poop in the middle of the night and decided to change her where upon I saw a significant amount of blood and mucus in her diaper. We called her pediatrician who asked if she has vomiting or a fever (both no at that point) and were told to bring her into the office in the morning. After hanging up we changed her diaper again (more blood now) and went to bed. 10 minutes in she vomited up what she had nursed and we called the ped again. She asked us to take her into the emergency room. With us went 4 bloody diapers that had a varying amount of blood in them the worst one had what appeared to be about 2 teaspoons of blood. At the hospital the doctor examined her rectum and found a small tear in the outside which he attributed to the bleeding. He also had to do a rectal exam with his entire pinky -- she hated that and I was one second away from smacking his hand out. They also did a culture on her poop, which we should get results from soon. As of tonight the bleeding was minimal (knock on wood) and sometimes non-existent, and she is now sleeping. Has anyone every had this happen and can breast milk cause constipation? That is how the doctor said she got the tear . . . from straining to get out her poops. I have only noticed soft breast milk poop, never hard poop. She does have oatmeal mixed with breast milk, but the poop is still soft. The vomiting she did that one time he said was unrelated. Other possibilities we were told: animals giving her a bacterial infection, Meckel's Diverticulum and some condition where the intestine inverts. She goes to the ped tomorrow morning for a follow up. We are sick with worry! Any advice or information will be greatly appreciated!

So What Happened?

Here is the latest. . .
By later that day the blood had minimized to only a few drops and we made an appoitment with her pediatrician for the following day. The ped heard us out and did not concur with the ER doctor that that amount of blood and the appearance of the mucusy stool was caused by the rectal tear, so she recommended a specialist (pediatric gastroenterologist) to see her. We saw him that same day and here is his conclusion. Breastfed babies do not get consitpated therefore there was another cause for the blood. He asked if I had a milk allergy as a child, and I told him I did. He asked me to describe the poops she was having up until the blood incident and I told him they were green. He asked if she had been on antibiotics recently, and she had been for sinus infection. Basically it turns out that because I had had a milk protein intolerance as a child that she had a 1 in 3 chance of having it as well. I have been eating a lot of dairy products and apparently the milk proteins go right into the breast milk and my daughter was getting them though me. She was not able to digest the milk protein and the result was that the bile in her body that aids in digestion was not able to take care of the milk protein and remained mixed in with the food as it went through her intestine. Bile is an irritant, in the intestine and outside on the skin, I bring that up because she had some nasty diaper rash quite frequently, so her intestines were being exposed to this irritant for months (as long as I was eating dairy.) The bile is also the reason she had the small tear in her rectum -- it irritated the skin there enough to make it tear -- specialist said that is a classic sign of a milk protein intolerance in breastfed babies. Once she started taking the antibiotics it killed off all the good and bad flora in her intestine and the result was that bacteria that normally are kept at bay were not and they proliferated in her intestine and most probably caused further irritaion and the resulting bloody stools. Not knowing any better, in the end it looks like I set my poor little bugger up to have this reaction (and I feel very guily about that). We still need the results of the bacteria cultures but that won't be until next week. As for me I must be dairy free and beef free, nothing in the cow family at all, which is OK with me . . . anything for my little girl. At 10 months we go back to the specialist and begin slowly introducing dairy into her diet to see if she is over the milk protein intolerance. So the short of it was: it was the dairy . . . :(

Dairy products are the number one leading cause of digestive problems in newborns - followed a close second by gluten - its hard to eat anything in this country without running into one or both of those. I am amazed that her intolerance to dairy proteins wasnt more pronounced...I have heard of days old babies having this happen to them. Within three days all the dairy proteins will be out of your system. Word of caution...dont replace all your dairy with soy based products, too much can effect (dwindle) your milk supply. Just remember its temporary...and its a good thing that you were nursing - (take a look at the prices at Nutramagen sometime!)

I know how you feel. When my son was about 3 months he also has that problem. I noticed a very light streak of blood initially and was told by a relative that sometimes babies pass a mucus like streak of blood when they have a cold in their system they are getting rid of. By the next bowel movement i did'nt notice anything. Then i saw it again later on that night and the next morning. I called the pediatrician and i too brought the diaper with me. I was told that it can be caused by straining eventhough his stool is soft. My little one has always grunted when going to the bathroom anyway. She also said that he could have been born with a tear and it went unnoticed. She did notice a little something when she was doing the same rectal exam. She said it looked as if he had a small tear that had already begun to heal. She took a culture because it could be serious and she did not discuss how serious because she did not want to alarm me unecessarily. I was told to keep an eye on it and wait for the results. Fortunately the results were negative and i just used bacetracian and A&D ointment on the red of his bottom. He had a tiny bit of blood in his stool later on that night after the doctors visit, but never again. By the way i was breast feeding as well. I had thought that maybe my breasts were cracked or cut somehow and maybe he was getting blood from my breasts. Girl i had all kinds of thoughts going through my head. Wow I am so sorry to hear your night was a true nightmare. I hope things go well for you and baby as well as your hubby. I was not happy to hear how they had to exam the baby so tiny like that. But I really hope you get good results back. I have 3 teens now I went through constipation, fever, flu. bumps and bruises etc. I will put a prayer in for your Family. Constipation can happen to any baby, regardless of breastfed or bottle fed, but like you said their would be hard poop. If its just the squishy stuff, then she's not constipated. A tear would cause bleeding, though. If you are concerned about constipation, talk to your ped about giving her a sippy or a bottle with some water or very diluted apple juice. Just make sure you give it sparingly, and at off times from nrusing. You don't want her filling her belly on that and reducing her breast milk intake.

I don't know what some of those other conditions are, but they sound unpleasant. Has she been crying a lot? If she had a bacterial infection, I would expect there to be a fever somewhere.
