Let's look at diet first. When there are this many, you need to seriously consider removing all milk/casein from his diet as that is usually the culprit. Don't replace it with soy as that is even worse than dairy.
I've been this route - the ENT (can't remember ours name, located in NB by Hoag) will say they need tubes. You'll get the tubes and there will still be problems.
Does your son have any other issues like eczema? With sleep? Bowel Movements?
There are so many options of how to replace dairy. I always send people to www.tacanow.org to the GF-CF Diet section as it has amazing information on what to do when you need to eliminate foods. There are recipes, where to shop, what to look for with hidden ingredients, etc.
Try it for one month. If it isn't making a difference then you need to look at what else does he eat. The most offenders are: dairy/casein, soy, eggs, wheat/gluten, corn, yeast.
BTW - with all of the antibiotics that he has probably been on, have you supplementing and adding in probiotics? Go to Sprouts, Henrys, Whole Foods and talk with the person in the vitamin section and have them help you.
