I'm sorry I don't know of any dermatologists, but my son had a heartbreaking diaper rash more than once. Ones that caused him to scream and cry, and that bled - is this what you are experiencing?
The pediatrician prescribed a medicated ointment for him, but it just kept him damp and didn't seem to help. What ended up working was cleaning him by squeezing out a warm washcoth on him and blotting it dry with the softest towel or blanket possible. (The water seemed to sting him, but it was a lot less abrasive than the wipes.) With the pouring of the watter and the blotting, there was really no friction or wiping to irritate him even more. Once he was dried off, I covered him front to back up to the hips with corn starch - really made sure absolutely every last bit of moisture was gone. I left him without pants as long as possible - espeically at nap time. It did lead to messy beds but the air on his skin for that length of time really made a difference within two or three days.
