I would probably take him to a different doctor and get a second opinion.
My son has had diaper rash on his anus for about 3-4 weeks. There are red, raw looking spots around his anus but no where else on his diaper area. He has BM every other day, sometimes every day. But he also has small BM a few times a day that amounts to about a tablespoon (he has frequent stool "streaking" in his diaper. I thought it was coming out when passing gas, but saw him actually poop this amount the other day when I was changing his diaper. He wasn't passing gas at this time). I think these frequent small poops are what's causing the rash to stay so persistent. Our pediatrician has us using hydrocortisone ointment on the anus every diaper change (every 1.5 to 3 hrs) along with vaseline on top of that as well as on his butt cheeks and under his scrotum. It is not getting any better. She said it's not a yeast infection so he doesn't need something like Nystatin. Has anyone had a rash like this and what did you do to heal it?

If it's not yeast, Paladin is really good. It's OTC, but you might need to ask the pharmacist where it is.
I remember when babies were diaper ed in cloth. Having a rash was common. No one worried about it's cause. They treated the rash as your doctor recommended And did not expect the rash to clear up in just a couple of weeks. My mom left her babies naked as much as possible.
I also think that the rash is in that location because he leaks a little bit. If you didn't discuss that with his doctor, I would give him a call. I suggest his bowel situation could be important.
Perhaps his diet is part of the reason. Mom's 50 years ago did try reducing fruits and spicy food. Too much fruit (acids ?) can cause the feces to irritate the skin.
Healing will take time. Or perhaps something else is causing the rash.
My son had a diaper rash and the pediatrician recommended maximum strength Desitin, which contains 40% zinc oxide. I used it after every diaper change and it significantly improved the rash. The only thing is that it has a very strong scent that I don't like, but I think maximum strength Desitin is the only diaper cream out there with 40% zinc oxide. I bought the big jar and after it was gone, I went back to using Triple Paste, which is scent free.
