Medical Power of Attorney

It depends on what you want it for. If you just want to give permission to someone to take your kids to the dr or hospital in case of an emergency while you are away, all you have to do is write on a piece of paper that so and so is authorized to get medical attention for your kids in case of an emergency and sign it in front of a notary and a witness and have the notary notarize it. Otherwise you may want to contact an attorney.

My mother is POA for my grandmother, and she had to go through a lawyer, just to make sure that she was applying for exactly what Grandma wanted.

Although I didn't check before responding here, you may be able to download a generic form from the internet. We had several different legal medical forms for my father and I am pretty sure we got some of them online.

I am pretty sure you can get the papers to fill out from your doctor or your local hospital. My father had them and there was a whole folder of info and variations of ways you can fill the papers out to suit your own case!! Goodluck
