Put a piece of scotch tape over his anus before he goes to sleep. In the morning if he has pinworms they will look like white tiny worms on the tape.
If he does you will have to treat him for the pin worms.

Temporarily until you can figure it out, try using a "butt cream", Desitin/Zinc Oxide ointment, or even vaseline, may calm down the itch.
Is he by chance constipated and has a little "ball" of stool that he can't get moved out? Constipation can cause loss of appetitie too.

I would need more information. Does he have a fever? Has his picking gone on for a long time, or only a day or two? Could he have eaten anything recently that might cause an allergic reaction?

Pinworms usually cause irritation or itching at night, not usually all day long. But many different kinds of problems can throw off a child's appetite, including normal variations in appetite from week to week. And many different kinds of problems, including a new nervous habit, might account for his picking or itching at his bottom.

If you're keeping the area clean and he still seems distracted, it would be a good idea to call the nurse at the pediatrician's office and ask her advice.

Use the tape test method. Put scotch tape around his anus when he goes to bed. When he wakes up , first thing, take tape off and examine it. If he has worms, you will see these white tiny little worms on there. Kids get them , and it is just a simple trip to the doctors, and they will give him some medication to get rid of them. I would call the nurse and discuss with them about his symptoms. They should be able to help you proceed.

He could be allergic to his diapers or wipes or his bath or laundry soap. Or he could have pinworms. Or he could have rectal strep. Or it could be from not so good hygiene (nothing against you, kids are just dirty little creatures). He could also be constipated. It really could be hundreds of things, but you really need to go to the doctor to have him checked out. In the meantime, cut his nails short and make sure he is washing his hands frequently. Good Luck!

If he doesn't have a fever, I suggest he isn't sick. It's common to have an itchy rectum from time to time. I'd just keep an eye on it and see if he doesn't stop picking at it. Be sure that the area is kept clean.

Do you use wipes when changing his diapers? If not, start that. If you do, try not using them and use a damp cloth instead.

give him a warm bath to soak in then put some cream on his bottom. Maybe a desitin type cream that has a bit of a tingle in it. maybe a hard or loose poop over the last couple of days made it irritated.

I would call the pedi and be seen just to be safe... if you think it's worms that is a doctor thing.

Pinworms. I had them as a child, from my dog. Went to a naturopath/homeopath and he gave me an injection of some remedy...gone forever. We use a homeopathic for parasites. ParaPro. It's a spray. 5 squirts a day until the bottle is gone may help a ton. Also, it could be yeast, so PB8 daily. We open the capsule and dump the contents into our kids' mouths. Think about how itchy you can get with a yeast infection vaginally. You might do both...since parasites and yeast can go hand in hand.
