Someone I knew had matching t-shirts made up for her kids saying "I'm the big brother/sister" in big colorful lettering and had them all wear it the next time the grandparents came to visit. The grandparents got the hint pretty quickly and the kids then had a shirt to wear once the baby arrived.

I have seen those t-shirts you can get for kids that say "I have a secret" on the front, and on the back it says, "I am going to be a big brother/sister". Those are always cute!

What I did was I had a double sided picture frame. In the one side was a picture of me, my husband, and our daughter. On the other was a piece of paper that said:
This frame: $5.88
This picture $.18
Knowing that there are actually FOUR people in this picture: PRICELESS

My M. and dad LOVED it!!

Congrats on your new baby!!

Write on 2 pieces of paper or poster board (cut in half) something like - I am going to be a big sister/brother in March 2010 or whatever date you are due. Then take their pictures holding the signs. Either upload and print them at Walmart (10 cents) and send to the grandparents OR put them in a cute email card (I love Smilebox - it's free too!) and email it to them with the uploaded photos of your 4 and 2 year old holding their signs or sign.
Good luck and congrats!

When I was pregnant with #3 I too wanted a unique way to tell the grandparents. I had t-shirts made that said "I'm going to be a big brother in May 2005" and I had them wear them when we went over to their house. My grandma screamed when she saw it lol she was so excited! We had them wear them to church too. :)

We had DS wear a t-shirt that said "I have a secret" on the front and "I'm going to be a big brother" on the back. We saw one set of grandparents in person and emailed photos of DS wearing the shirt to the other set (and other family members).

We had a T-Shirt made for my daughter (at the T-Shirt Deli in the city) that said "Big Sister coming June 2009". We had her wear it to Thanksgiving. It was great because my mother-in-law is ALWAYS commenting on how cute-ly she's dressed and it was nice to see that she really DOES pay attention! It was great! We had her wear it again to the huge family Hanukkah party and for my parents as well. Worked great!

When I was pregnant with our second we bought the book "I'm going to be a big brother" by Brenda Bercun. (She also has a big sister version) To tell my parents we had our son ask Grandma & Grandpa to read his new book to him. It was a fun way to tell them, and as baby's arrival got closer it was a wonderful tool to use with our 2 yr old to talk about the new baby coming.

We put our 2 1/2 year old in a 'big brother' t-shirt and then just waited for Grandma to notice. She was totally excited, her mouth fell open, and all she could say was "really?!?!? really?!?!" then raced across the kitchen and practically tackled me, trying to hug me.

Yes, I know, not too creative, but certainly effective!

(For the first kid, we put the positive pregnancy test in a plastic baggy then wrapped that. When they went to open it, they had no clue what it was, started to open the plastic baggy when we had to call out "Oh no!" and then had to explain it to them)

Edited to add - Michelle P's idea sounds really cool!

I had t shirts made for me and my son on a website, www.cafepress.com. My son's said "1 of 2" and mine said "2 of 2" with and arrow pointing down. We wore them on a visit and they got the clue. It was cute.

I got a hamburger bun and put it in their oven. Then the conversation went like this (me first): "What are you cooking?" "Nothing." "There's something in there." Let them get it. They actually said verbatim "Why, there's a bun in the oven." Great to watch the realization come over their faces! Too funny!!

When we wanted to tell the grandparents that we were expecting a second baby, we let me son do it. He was just about 2 then and when we went over to their house for dinner about halfway through the dinner I made a fuss that he got food on his shirt and that I needed to change him. I had a shirt that said Big Brother on it. After we got back, it took them a bit to notice. But their reaction was great when they did!

Also, I had wanted to try this out if we had a third, so you'll havve to let me ko=now if it works. Fortune cookies! I have heard that if you put a fortune cookie in the microwave with a bowl of water that you can open it and switch the fortune. I think that would be so great!
