I have a letter I have used for both public and private schools. I have never had a problem and my oldest is in 7th grade. If you want me to email you the letter just let me know :)

I'm trying to prepare for when my child enters school next year he doesn't have vaccines and I've done my research and I feel that it's in the best interest for my child to be vaccine free. I know in Illinois you can get exemption from vaccines but I don't know how to go about it. Who or where do I go and so forth, He will be going to a private school so I don't know if the requirements will be any different then public school. If you have any experience or can direct me, it will be greatly appreciated. Thank You

Go to National Vaccine Information Center (www.nvic.org) they should have all the forms/letters you need to fill out and have signed. I haven't looked the site in a while, but I know you can find a generic exeption letter that you can use and personalize.
All you need to do is print one of these and send it in. I know tons of families that use it. PM me if you need more info.
