I would contact AZ Department of Health Services. I know there are free evaluations performed year round for children over 33 months, but can't find the info right now. DHS should be able to point you in the right direction. Good luck!

well thank you very much for the responses i got. i need to get my daughter evaluated for speech and behavioral issues. NOT THAT LONG AGO SHE WAS PLAYING A GAME ON THE COMPUTER AND I STARTED TO NOTICE THAT SHE WAS SITTING TOO CLOSE TO THE MONITOR. WE WERE PLAYING A MATCHING GAME WITH LETTERS. WE TOLD HER TO SCOOT BACK, WHEN SHE DID SHE WASNT ABLE TO MAKE OUT THE LETTER. NOW THATS ANOTHER WORRY, BUT IM GONNA GET ON THE BALL AND FIND HER SOME HELP. ONCE AGAIN THANKS A BUNCH EVERYONE FOR YOUR TESTIMONIES AND AVICE.

Not sure what you want to evaluate her for, but I have learned through trial and error that the psychologist label is not helpful and even more so, meaningless! I done the evaluation route only to be told that my son had ADHD and needed meds. I refused, as he was only 5 at the time. I was told that I could spend millions on therapy and it would not work, the only thing that would help is medication. Again, I refused and told the shrink that would take my chances with therapy. Then I found the most amazing therapy that completely turned my son around. It was vision therapy. Turned out that he did not have ADD/ADHD, but instead had an eye teaming issue. They worked with him to get his eye aligned properly and he is a completely different kid! Happy and loves to read and do school work! Take your daughter to be evaluated by a behavioral optometrist. Send me a note privately letting me know where you live and I will find one for you. I am so impressed with my sons progress that I have gone to work for my eye doctor to help to spread the word about vision therapy. It was the miracle that I had be desperately searching for! I highly recommend it! I can send you the article that we have written about the correlation between vision and learning. Good luck!

what do you want her to get evaluated for???
If it's for delays with Speech, Language, Motor Skills, Social Development - call your local school district and they can evaluate her. You may also go out of district for this too. I'm in the Kyrene School District and my boys have started with their preschool program since they were 3 for Speech and Language.
There is a program in Arizona called Arizona Early Intervention Program (AZEIP). At age 3 those children being helped by AZEIP are transitioned to the school district for continued services. Contact your local school district for an evaluation. Evaluations are done several times a year and they can offer services free of cost to you, if needed. I have 2 kids who have been helped through AZEIP and one is now attending a special pre-school for developmental delays through the School District. It has helped them so much!! Good Luck!
I guess it depends on what type of evaluation you need. If you need her to be evaluated for speech therapy or motor skills development, contact your local school district and ask them when and where the next screening is.
