It depends on the daycare. Some have forms that they want your doctor to fill out. The main thing is that you child is healthy and that the doc. reports this, even if you just have him write this on a prescription pad, it's usually sufficient enough, and the daycare can make a copy. Also, all daycares require immunization records. The best way to handle all this is to go to the dr. for a well check, if you have not had one recently, and tell him you need a note to get you kid into daycare, and also get a copy of the immunizations. If you had just recently had a well-check, save yourself the trouble and just call the doc.'s office and ask if he'd be able to write the note from the last checkup and you can just pick it up without an appt. But make sure you check with the daycare to ensure that the last visit was within their valid time frame. Good luck, and I hope this help.
