Treatment Release Forms?

My parents will be taking two of my children on vacation with them. Does anyone know where I can find medical treatment authorization forms online? Thanks!

So What Happened?

Thanks ladies. The forms have been found and signed. The middle two kids are on their way to New Mexico with my parents. My oldest leaves for church camp on Monday. Hubby is off for the entire week, and we'll have an only child for 5 days :)

No forms needed. Simply write a note stating that you give them full authority to make any and all medical decisions for child #1, 2, 3 etc.(exact names)and that you and hubby take full responsability for the cost incurred. Both of you sign and date. I always give them either the medical ID card or include all pertinant info on the back of signed note.

Email me at ____@____.com, when I get home tonight I will forward you the one I used. It is a word document. You have to have it notarized in order to be legal, but you can do it at your bank or find one at work or locally. Just don't sign it till you get there. -J. White

It's a good idea to complete one. A number of years ago I put one together that has much of the same information as the one that Lisa linked you to. What I have in addition are contact numbers, insurance information, and doctor information. I actually complete one for each of the sitters that I have for my children. I don't have them notarized, but I think the authorization is pretty important.
