You've gotten some really great advice so far. I'm a dog behaviorist and a lot of these moms are right on with the fact that you trained her to go potting indoors and she doesn't understand that this is unacceptable.
I'm guessing that you either leave her in the crate for too long, or you got her from a pet store for her to go potty in her crate. Most puppies will NOT do this unless they have no other option or they have been conditioned to do so.
A few things I would add...keep her with you all the time by leaving a lightweight leash on her to drag around, take her out (on the leash) very frequently on a schedule through the same door every time, to the same spot everytime. Stay with her and watch her go potty, and priase her when she's actually going not after she's done. Avoid playing any games with her in this area. Give her 5 min to go potty and if she doesn't take her back in the house, to her crate leave her there for about 10 min and take her back to the same spot outside. Repeat this process until she goes potty. Schedule her food AND water so you know better when she'll have to go and make sure you are NOT leaving water down for her to drink whenever she wants it. It may be that she she's drinking WAY too much water and has to go very freqently. She should be drinking about an ouce for every pound that she is and in hot weather drinking about an ounce and a half.
DO NOT rub her nose in it or punish her when she does go inside. She won't understand this and it will only cause her to fear you. Instead paise her very generously as she is peeing outside and get out more freqently to go.
You should expect this will take about 2-3 months to re-train her before you can trust her to be on her own all the time again.
